Proof of Identity required on the day
You must provide proof of identity POI either by:
- One Full Proof identity document, or
- One Primary ID plus One Secondary ID, proof of identity document
Acceptable Full Proof POI
- Photographic Australian drivers licence (current)
- Australian passport
- Document of ID issued by passport office
- ADF ID card
- Australian police officer ID card
- Police warrant card
- Proof of age card issued by DoT
- Marine cert of competency (photo)
- WA security licence issued by WA police service

- If you have changed your name, you must provide one of the following:
- Marriage certificate
- Divorce papers
- Change of name certificate, deed polls or licence to change name documentation
Acceptable Primary ID
- Overseas passport, not more than 2 years out of date
- Photographic Australian drivers licence, not more than 2 years out of date
- Australian citizenship or naturalisation document issued by DIMIA or passport office
- WA firearms licence
- WA Motor Driving Licence learners permit
- Birth certificate or certified copy
- Consular ID card issued by Dept Foreign Affairs & Trade
- WA security licence , not more than 2 years out of date
Acceptable Secondary ID
- Entitlement card (Medicare) (current)
- Bank card (current)
- Bank passbook, not more than 12 months old
- Water or local rates notice, not more than 2 years old
- Electoral card or equivalent, not more that 2 years old
- Certificate of accomplishment from educational institution
- Letter from principal of an educational institution, not more than 12 months old
